You donââ¬â¢t need to dream up a cure for cancer or a new way to bring water to the developing countries. Think about a challenge that faces the ones you love every day - you should pick something that feels real and honest. Donââ¬â¢t try to impress with size and scale here. Instead, make a small, meaningful impact. If youââ¬â¢re able to describe how your project really makes a change, it will be more memorable in the eyes of the admissions committee. School of Architecture - Describe an instance or place where you have been inspired by architecture or design. Inspiration comes in many forms - whatââ¬â¢s important for this prompt is to think about what has inspired you, not the masses. In other words, donââ¬â¢t feel like you need to pick the Empire State Building if you werenââ¬â¢t actually inspired by that work. Pick something that was meaningful to you and expand on why it impacted you. You could even pick a small home - something that encouraged you to think of building dwellings that are important for families. Whatever you choose, you should be sincere about your inspiration. Anything that sounds trite will be really obvious to the admissions committee. Saying something like ââ¬Å"I want to solve cancer for everyoneâ⬠is less impactful than saying that you have seen firsthand how cancer affected your grandmother. Nursing is a personal profession - lean into that for this essay. Kinesiology Program - Discuss experiences that led you to choose the kinesiology major. For this prompt, focus on one or two experiences and discuss them in detail. Donââ¬â¢t give an entire overview of your history - describing something more fully will resonate more than trying to cram a lot of experiences into a relatively short essay. Be honest about what drove you to kinesiology - donââ¬â¢t write what you think the admissions committee wants to hear. Write whatââ¬â¢s true for you. UVA Prompt #2: The instructions are the same for all of the second round of UVA prompts: "Answer one of the following questions in a half page or roughly 250 words." You only need to pick one of these questions to answer. Whatââ¬â¢s your favorite word and why? With a prompt like this, it can be tempting to want to impress the admissions committee with your knowledge of esoteric vocabulary. Donââ¬â¢t go that route, unless you have a true connection to a strange, rarely used word. Pick a word thatââ¬â¢s meaningful to you - something thatââ¬â¢s very specific to you. The key here isnââ¬â¢t to pick the most interesting word, itââ¬â¢s to have the most meaningful and memorable experience to back it up. If ââ¬Å"familyâ⬠is really important to you, you need to have a good reason - simply saying you love your parents isnââ¬â¢t enough. If you want to talk about Kim Kardashian, thatââ¬â¢s cool - as long as you can indicate why this is a topic worthy of discussion. UVA students paint messages on Beta Bridge when they want to share information with our community. What would you paint on Beta Bridge and why is this your message? As with the other prompts, the ââ¬Å"whyâ⬠is the most important part of this question. Whatever message you land on, make sure you have a good reason for it. Avoid trite or cliched phrases, like ââ¬Å"Be the change you wish to see in the world.â⬠The admissions committee will have seen thousands of these - and those words are probably already written on Beta Bridge. What do you have to say? What message is personal to you? What lesson have you learned that you specifically can communicate? UVA students are charged with pushing the boundaries of knowledge to serve others and contribute to the common good. Give us an example of how youââ¬â¢ve used what youââ¬â¢ve learned to make a positive impact in another personââ¬â¢s life. First and foremost: donââ¬â¢t make something up here. Donââ¬â¢t say that you spent months devoted to helping a family whose father was suffering from cancer if thatââ¬â¢s just not true. While this prompt does encourage you to share how youââ¬â¢ve served people, itââ¬â¢s not a contest. You donââ¬â¢t have to have the grandest and most impressive example of selflessness that the committeeââ¬â¢s ever read. What you do need to have is sincerity - talk about how you helped someone and why that experience was so impactful. Explain what led you to help and reflect on how UVA might inspire similar feelings of service in you. How to Write UVA Essays Here are some general tips for how to write UVA essays that will wow the committee. #1: Be You Your UVA supplement essay is a chance to show the admissions committee who you are. Take that opportunity to flesh yourself out. Youââ¬â¢re not simply a collection of Aââ¬â¢s and Bââ¬â¢s printed on a transcript. Youââ¬â¢re a real person! Manual Vacuum Aspiration for First Trimester Pregnancy Loss Dr. Sindhoo TO DETERMINE THE EFFICACY OF MANNUAL VACUM ASPIRATION IN MANAGEMENT OF FIRST TRIMESTER PREGNANCY LOSS INTRODUCTION: An estimated 46 million abortions are performed globally each year, with one-third of all abortions are performed under unsafe conditions leading to 13% of all maternal death1. 10ââ¬â12% of these abortions are performed in the early first trimester1, 2. Local data shows an annual abortion rate of 29 per 1000 in women aged 15-49 years. Incomplete and missed miscarriage being the commonest occurs in approximately 15 % of clinically recognized pregnancies and in 890,000 women per year3. One study conducted by Shonali Agarwal and Dolly Gupta reported the efficacy of MVA 90% in management of first trimester pregnancy loss5. First trimester of pregnancy ranges between 5ââ¬â13 weeks and is associated with 3 to 5 times lower maternal mortality and morbidity risks than termination during second trimester4. Although the etiologies of first trimester loss are multi factorial and often remain unknown, certain risk factors increase the likelihood of pregnancy loss. Findings such as absenc e of cardiac activity after 5 wks, gestational sac smaller than expected for dates, an abnormal-appearing yolk, intrauterine hematoma (sub chorionic hemorrhage), vaginal bleeding after 7 weeks of gestation are potential markers for subsequent pregnancy loss4. Once a first trimester pregnancy loss has been diagnosed, there are three forms of management: expectant, medical, or surgical evacuation (with electrical or manual vacuum aspirator). The optimal mode of management is determined by gestational age, whether the pregnancy loss is delayed or incomplete, maternal hemodynamic stability, the presence of infection, and, most importantly, patient preference5. Manual vacuum aspiration (MVA) employs a vacuum pump in which the vacuum is created using a hand-held, hand activated plastic syringe. 4,5,6 Manual vacuum aspiration is safe, effective, easy to use, portable, and reusable. It is appropriate for use in many different clinical settings (Including office, emergency room, or the operating room) thus avoiding the need for the use of an operating theatre and the risks of general anesthesia. It does not require lengthy training for proper operation, and may be performed by a wide range of trained medical personnel including midwives and nurses. When conducted in the outpatient setting rather than operating room, manual vacuum uterine aspiration can result in substantial cost savings, electrical power saving and significant reduction in procedure time (3.7 minutes for MVA vs. 10.2 minutes for DC).The World Health Organization (WHO) recommends MVA as a preferred method of uterine evacuation2,7. RATIONALE: On robust literature search there was scant relatively old data available on efficacy of MVA but no one focused on efficacy of manual vacuum aspiration in management of first trimester pregnancy loss. This provide me the strong rationale to conduct this study, the aim of my study is to set a baseline data which will help to generate the current local statistics of MVA in terms of efficacy on this particular group of women with loss of pregnancy. If we found the significant efficacy of MVA then we will recommend MVA as first line of treatment in future in management of first trimester pregnancy loss. OBJECTIVES: The aim of our study is to determine the efficacy of Manual vacuum aspiration in management of first trimester pregnancy loss. OPERATIONAL DEFINITION: FIRST TRIMESTER PREGNANCY LOSS: An abortion is the spontaneous or induced loss of an early pregnancy. Miscarriage occurs within 12 weeks of gestation will be confirmed on ultrasound. EFFICACY: If an empty uterus with no evidence of retained products of conception seen within 24 hours on Ultrasound will be label as efficacy positive. MATERIAL AND METHODS: SETTING: Liaquat university hospital Jamshoro /Hyderabad DURATION: Six months after approval of synopsis. STUDY DESIGN: Case series study. SAMPLE SIZE: By using WHO sample size calculator taken efficacy of MVA i.e P=90% (5) margin of error (d) = 5.5% confidant interval 95% then the estimated sample size will be at least n=115. SAMPLING TECHNIQUE: Non-probability consecutive sampling INCLUSION CRITERIA: All women with gestational age of âⰠ¤ 12 weeks confirm from last menstrual period with pregnancy loss (as mention in operational definition) Age 18-35 years Women with any parity Exclusion Criteria: Women who are hemodynamically unstable (systolic B.P Ectopic pregnancy assess on ultrasound Known systemic disease including hepatic or renal dysfunction. Had a history of cervical surgery. DATA COLLECTION PROCEDURE: All patients fulfilling inclusion criteria and not having any of exclusion criteria attending Outpatient department/emergency department of Liaquat university hospital Jamshoro after evaluation by consultant will be included in the study. Informed consent will be taken from each patient about procedure and this study. Approval of the ethical committee will be sought. Patients will undergo elective abortion. All operations will be performed by fourth year residents under supervision of consultant having more than 5 years of post-fellowship experience. Intravenous anesthesia will be used in all the cases. Manual vacuum aspiration will be performed under par cervical block with 10-20 ml of 1% lignocaine using Glick technique3. MVA will be performed using a flexible Ipas Easy Grip cannula attached to a 60 ml syringe (aspirator), with a double locking valve mechanism. To assess the efficacy(as mentioned in operational definition) of MVA. Patients will be fallowed till 24 hours. Demographi c variables of name, gestational age, and parity will be noted. The final outcome in terms of efficacy will be noted after 24 hours will be recorded on approved Performa. DATA ANALYSIS PROCEDURE: Statistical analysis will be performed using statistical software SPSS version 16 on computer. Mean à ± standard deviation will be calculated for age gestational age and parity. Frequency and percentage will be calculated for efficacy. Effects modifier will be controlled through stratification of age gestational age and parity to see the association with outcome i.e (efficacy) by applying Chi-square test taking PâⰠ¤0.05 will be considered statically significant. My Personal Theory of Counseling Essay
The important ingredients in counseling are: understanding of theory, process and skill. There are many theories of personality. Moreover, we will notice that within each theory, there are other theorists who slightly differ in their views from those of the exponent of the theory. More often, counseling is based on how the proponent of the theory views personality. Burrhus Skinnerââ¬â¢s behavioral approach did not go deep enough while; Sigmund Freudââ¬â¢s theory on personality went too deep. Gordon Allportââ¬â¢s theory of individuality and functional autonomy also lack the foundation I need. My personal theory is more on the source of traits in describing personality, are deeper and more significant. It is however different from the factorial theory of Raymund Cattell. I admire Carl Rogersââ¬â¢ self-actualization; however I believe a person is capable of it only if his brain is structured for self-actualization. Analysis/Discussion I was contemplating to structure my personal theory on counseling on the various theories that we have learned so far; however, I find it illogical to base my theory on another theory of unproven facts. I want a concrete foundation of my theory. Do you suppose inconclusive studies of Frued, Skinner, etc. provide solutions to problems? Definitely not! I have read some articles about Sigmund Feud and I doubted whether he had treated his clients with success. It is my belief that psychotherapy can only help to remove the symptoms and prevent early recurring of these symptoms, however when triggered in the future, the symptoms again manifest. .Professionally, I opted to base my personal theory of counselling on pragmatisms, that is, ââ¬Å"genetics. The conclusion of the study just completed by Dr. David Reiss on adolescent development was a surprise even to the doctor himself. Genetic qualities of a person make him unique and different from other people. It is the brainââ¬â¢s genetic components that determine the personality of a person and since no two persons have exactly the same genes, no two people have exactly the same brain structure. Even identical twins have different personality (ââ¬Å"How does the brain works? â⬠). Identical twins have different fingerprints when they come forth from a single fertilized egg. Personality does not begin during infanthood as theorized by Frued but at conception, that is, fertilization of an ovum by a spermatozoon, to be exact. Genetics influences proved that relatively, parents have little influence on child development, that genetic is responsible on how children do well in school, how these adjusted individuals turn out, whether they develop delinquent behaviors in their interactions with other people. ââ¬Å"Itââ¬â¢s a radical revision of contemporary theories of child development,â⬠says Reiss. ââ¬Å"I canââ¬â¢t even describe what a paradigm shift it is (Paul, A. M. 005). ââ¬Å" This is the reaction of a George Washington University psychiatrist, Dr. David Reiss as described in a recent article in Psychology Today Magazine, after receiving the results of his more than 12 years of study on genetic and personality development (2005). Researches say that the genetic composition of a person brings forth peculiar responses from others, which suc cessively shape his personality. It cannot be denied that a baby or a child with a bright and pleasing disposition attracts other peopleââ¬â¢s attention and affection, thus creating self-image and importance. Studies proved, children in their interaction with the environment would pick experiences and opportunities that befit their genotype or genetic qualities. They make decisions in his choices in life, like choosing his friends, jobs, etc which determine their personality as they grow older. The children are in control of their own development through the choices they made. The genes lay the foundation of personality; the environment provides the color and refraction, the accent and significance. That is the correlation of ââ¬Å"nature and nurture (2005). â⬠Moreover, where there is conflict between nature and nurture, problems arise. This happens when society does not permit or support the childââ¬â¢s expression of his natural propensities or where the environment is unable to provide enough opportunities to develop his inner potentials as when he was born to a poor family or where the childââ¬â¢s genetically- determined inclination collides with that of the parents. Such conflicts may result in anti-social behavior, diminished motivation and poor mental attitude. The philosophy of genetics is consistent with my view about personality and humanity. The world is a great plan, a program where everything has a purpose, an essence. The seas are what they are, to contain other living things to live. Animals survive by instincts and conditioning, no need to be counseled. A building did not exist without a reason. It was planned and erected for a definite purpose. Man came to existence by essence of choice. We are not totally free to get want we want, to act what we want to do. There is always a choice. We are only free to choose. If we want to fly, we cannot, because we are not programmed to fly. However, we are free to do something about it. However, what we willed is subject to the will of nature and chance. Chances are, our wishes and desires will not be realized. We choose our attitudes towards the things around us and give meaning to our life. We try to understand things in life but our understanding is limited. Being raised in a Christian environment, I find it in accord with my spiritual beliefs as a person, in the sense that before we came to this world, we already existed in the spirit with God. Allow me to quote a Bible verse, in Jeremiah 1:4-5: ââ¬Å"Then the word of the Lord came unto me, saying, before I formed thee in the belly I knew the; and before thou camest forth of the womb I sanctified thee, and I ordained thee a prophet unto the nations (cited in Richards, L. l979). â⬠This may sound unscientific; however there is truth to it. It is true that even inside the womb; the baby has already his own unique personality. Darwinââ¬â¢s theory of evolution until now remains a theory and every theory on personality continues a concept. Structure of Therapy and Choosing Goals In choosing goals, initial assessment is very important because this is where the problems lie and where the outcome of the treatment ultimately depends. The therapist must go deeper into the root cause of the problem. Freudââ¬â¢s structure of personality centers on the id, ego and superego which are interrelated and based his psychoanalytic treatment on this structure. Sigmund Freud missed the foundation of the id, the genes. My structure of counseling will be based on the philosophy of genes, that no people are alike. It shall be dependent on the nature of the clientââ¬â¢s personality. What a dilemma! I have to search for the genes that determine personality to go deeper, realization of which is next to impossibility. However, if science is unable to provide the truth about reality, philosophy can do that and spiritual beliefs do the same with the special job of construing the significance of otherworldliness within this structure. Before starting the therapy, I should consider determining the nature of the client. Knowing the client is a very important aspect of counseling. Of course I have to inquire about my clientââ¬â¢s childhood, if feasible talk with the mother or the baby-sitter, the father and siblings. Gather all the information about the client from birth to adolescent, his fears, traits, attitudes, beliefs, desires, likes and dislikes. From there, the nurturing of the client must be dealt with also. Collect information about her/his peers, playmates, school, teachers and the environment. More or less, there is now a clearer picture of nature and nurture. After the history-taking session, the source of conflict will now be easy to determine by applying the ââ¬Å"right brainâ⬠technique to the client. The ââ¬Å"right brainâ⬠refers to the brain systems that fix or adjust the power to interpret, direct and get ahead in the world and also ascertain the quality and security of relationships with others. Right brain counseling centers on emotion and creativity. To bring this emotion and sensations into play, the ââ¬Å"left brainâ⬠is used. The therapist must make use of both sides of the brain to optimize counseling. This is an approach to healing with a goal to balance the emotional, physical, moral, social and spiritual nature of a person called holistic and integrative approach. This is addressing a person as a whole in all aspects (Lacombe, S. McGraw, T. 2007). Both the therapist and the client must be comfortable with each other to arrive at a desired goal. The client must be made aware of the facts of life; that not everything we desire is attainable; that our understanding of the world is limited and that there is free will of choice; that we should not regret if we fail in our choice; that everything will pass naturally if there is acceptance. That there is always a chance to do better and the future is still fruitful and bright. That desires can be shifted to other endeavor. Learning produces alterations in gene expression. Genes contribute to behavior; in turn behavior feedbacks action on the brain and modify expression of the genes. ââ¬Å"Thus all of nurture is ultimately expressed as nature (Kandel, E. l998)â⬠However, I still believe that under extreme circumstances, nature predominates. Summary/Conclusion A psychotherapist must formulate his own unique standpoint based on profound precepts of knowledge. In doing so, it is necessary to be cautious about formulating a personal theory. Counselors must come to understand the nature of people and deeply look into their values, attitudes and beliefs about existence and good life. In going about choosing goals, a therapist must key out a substantially mature and functioning individual as a model. My personal theory of counseling is based on pragmatism that genetics provides the foundation of oneââ¬â¢s personality, nurtured by his experiences and choices thereby reflecting and defining his personality development. Problems and symptoms arise as the genetic traits or characteristics of a person are in conflicts with his environment, where nature and nurture do not coincide. In learning, expressions of genes are modified thus nurture ultimately is expressed as nature.

His poetry depicts his experiences in the greenhouse during his youth which begun in 1941 entitled Open House which won a Pulitzer Award and National Book Awards among others. He was also given the Bollingen Prize by Pennsyl vania University in 1959. Theodore Roethke was born in 1908 on the west side of the river in Saginaw, Michigan. His father who was a German immigrant and market-gardener owned a greenhouse together with Theodores uncle where he spent most of his time. This experience can be reflected by the way he used natural metaphors when delving with his poetry. His adolescent years had been jarred by the death of his father who suffered from cancer and the death of his uncle who committed suicide on same year in 1923. Theodore was only 15 at that time thus such unpleasant event had jarred his psyche and the way he had molded his creative life into the world of poetry. Roethke became eminent when he graduated with magna cum laude honors at University of Michigan, Ann Arbor in 1929. He stood against the family pressure for him to pursue his study in law and took his chosen graduate courses from University of Michigan and later at Harvard Graduate School and worked along with Robert Hillyer who was also a poet himself. The economic crisis during the Great Depression drove Roethke to shift from Harvard to Lafayette College and took a teaching course. He then met Rolfe Humphries who introduced Louise Bogan to him. These people became his avid supporters and colleague. They became friends to poet Stanley Kunits. In 1935 Roethke assumed responsibility in his second post in teaching at Michigan State College in Lansing but he was hospitalized due to frequent mental illness. Consequently, Roethke made use of these periodic recurrences of depression to explore a rather new level in reality through innovative self-exploration. As quoted by Theodore Roethke: The greenhouse is my symbol for the whole of life, a womb, a haven-on-earth unquote.
